The United States, England, Germany, and France have equivalent life expectancy rates, yet medical treatment differs enormously from country to country. A new foreword by the author examines the trend toward evidence-based medicine and addresses the substantial changes in medical culture since 1988, including the proliferation of alternative medicine and the changing face of medicine in the European Community since the fall of Communism.

About the Author

Lynn Payer, a former editor for the New York Times Good Health magazine, writes extensively about medical issues for doctors as well as for the general public. She lives in New York City.

A fascinating anthropology of European and American medicine

While we may be used to looking at the anthropology of less developed countries, Lynn Payer turns her lens at European and American medicine. In England one keeps a stiff upper lip, as doctors give fewer tests, less medicine and lower doses, even when not rationed. West Germans use six times the number of heart drugs as the French or English, although the three countries have similar rates of heart disease, often using several at once due to the attitudes towards the heart. France looks more at the terrain than the pathological organisms attacking it, strengthening the immunological system with techniques Americans would consider fringe medicine. And French doctors would use a hysterosalpingogram instead of the D&C that German, English and Americans use to diagnose conditions because they are afraid of adhesions from surgery that might impair fertility. American doctors do excessive hysterectomies that would be considered unwarranted in England, France and Germany and, at least at the time of the book, radical mastectomies instead of lumpectomies.

The book also looks at how medical compensation affects the way medicine is practiced. German and American doctors who are paid fees for each procedure use far more than English doctors who are paid a straight salary. American doctors may raise their fees when they want more compensation while French doctors would need to perform more operations. American doctors whose insurance companies would require them to perform a cesarean section after fibroid removal are more prone to remove the uterus than French doctors who have no such pressure and feel that a woman could have six myomectomies before a C-section would be required.

And different organs are looked at as important. The French place great stress on the liver, its ability to process food and to regulate the body while the Germans focus on the heart.

While we may consider medicine a science apart from cultural considerations, Payer shows us that the art of medicine is subject to cultural and economic biases even in the modern European-American context.

quite good, but a bit old info

Having experienced medical care in five different countries, according to my experience varieties in treatment in different countries are very common. Though former British colonies still retain lots of resemblance to British health system. My only objection to the book is that it needs an update - lumpectomies instead of radical mastectomies are getting more common in US. However, hysterectomy, often unjustified, is still far to common in US with no hope for change in near future. Also, doctors seem to be unable to understand that different countries have different disease statistics even after you bring them articles printed in medical journals proving that you are right. Medicine does not deserve to be called science, IMO.

The spirit of medicine comes from culture

In Medicine and Culture, Payer describes what she sees as the spirit of the medical system in France, Germany, England, and the US. I say spirit, because she writes about the feel she got for the medical system in each while doing business and living abroad. She doesn't detail how the medicine is financed and regulated, and statistics are presented sparsely to illustrate points, and not flung at the reader. She is very much writing about the feel of medicine and how it interacts with culture in each country. This makes the subject matter less dated, because culture changes slowly, unlike specific medical procedures or government regulations.

I read this a few years ago and I feel that it has given me some important perspective and insight into medicine, which helps me interpret other information that I get about medicine. It was a fast read and worthwhile, even if you came across the book while looking for a more concrete treatment of medicine in different countries.

My patient’s name is Joe, or so he says, and I am his nurse. His chart states his name as Zbigniew Zwiezynskow and under place of residence, there is merely a sad trio of letters: NFA — “no fixed address.” He’s in his mid-forties, admitted last night to the Intensive Care Unit, the ICU, with pneumonia. He’s feverish, delirious, and so violent that he may try to kill me if I decide to release him from the restraints we’ve placed on his arms and legs. Medically, his condition is improving — no small thing here in the ICU, where all of the patients have life-threatening, catastrophic illnesses. We’re full; each of our twenty-two beds is occupied. Attached to every patient are monitors, machines, and equipment and in every room there are sickening odours and horrific sights, but I hardly notice these things any more. I have learned how to go beyond it all, to see through it, push it gently aside and go straight to the person lying there in the bed.

Today, all day, Joe is my patient. His heartbeats, urine output, breaths, cough, skin, dirty fingernails, and wild, greasy hair are all my concern. I will enter his world and for the next twelve hours, minute by minute, I will dwell with him there.

It’s taken me such a long time to get here.

The sheer math astounds me. Since becoming a nurse in 1983, I have put in thousands of twelve-hour shifts at many different hospitals. I have worked with thousands of nurses and hundreds of doctors and other professionals. I have taken care of tens of thousands of patients ranging in age from eight to 104 who have had a multitude of diseases, illnesses, and injuries, and have administered to them a sea of intravenous fluids, rivers of syrups, suspensions, injectable medications, and at least a million pills, capsules, and tablets. Researchers study large populations, searching for patterns and trends, but the only way I know how to practise as a nurse is one patient at a time, seeing each individual in my care through illness, loss, pain, grief, or the prospect of death. For me, each time, the numbers all come down to one. As a nurse, there is the patient I am caring for and together, we proceed, one on one, side by side.

It wasn’t always like this. Even though I was practically born a nurse, with strong instincts to help others, I was raw and unskilled; I had to be made into one. But long before it became my livelihood, taking care of others was my way of life. You could say I practised as an amateur for many years before going professional. My mother was my first patient and I cannot recall a time when I did not know it was my job to be her nurse. I was six years old when she first became ill, but as my father always said, I was very mature for my age.

The Making of a Nurse is the true-life story of author Tilda Shalof, a caregiver all her life, and an Intensive Care Unit nurse with twenty-five years of experience in Israel, New York, and Canada. She tells stories of overprotective "helicopter parents", so named for their tendency to hover over their children; dealing with patients ranging from gracious to delusional or even violent; to the virtual war between the mother and a girlfriend of a dying hospital patient over his comatose form. An absorbing and revealing look into the day-to-day life of a nurse, as well as the personal travails - including a stint of life on the streets - that beset the author. Highly recommended.

Good motivation

In my second year of nursing school I had a very hard time in one of my clinical rotations. I was having a difficult time believing in myself and whether or not nursing was right for me. At one point I considered dropping out. This book made a huge impact on me as Tilda had similar challenges as myself, and I realized that nurses are all made up of various strengths and weaknesses. I read this book over 6 years ago and it will always have a special place for me. I recommend it to anyone but nursing students have a lot to gain especially by reading this.

Tilda Shalof's autobiography

I read this book with the high hopes of gaining insight on what it takes to be a nurse. While Shalof touches on this subject, she fills the book with her personal life outside of nursing such as her travels, family and her coworkers sexual encounters. I found the book to be dull, boring, and often far,far away from anything having to do with the making of a nurse. I really felt this book could or should have been Tilda Shalof's complete autobiography, in which nursing is simply one of the many elements.

An excellent review of the Mexican-American music scene.

This book represents a scholarly analysis of music forms that are familiar to many Americans, yet whose origins are not always known. Mr. Loza obviously knows the material on a first-name basis and sets it out in painstaking clarity. Also included are interviews with notable purveyors of popular Latin music (Los Lobos, Poncho Sanchez, Lalo Guerrero, et al). Adorable models

No Starch Press’ latest LEGO book is adorable. The LEGO Christmas Ornaments Book by Chris McVeigh contains 15 Christmas themed models in two groups. The design of the geometric hanging ornaments is really inspired. The second half has instructions for several micro-objects, like an old-school arcade machine, an tiny desktop PC, and a SLR style camera. Adorable.

Like most of the things No Starch Press does, I love this book. The photos are gorgeous and the book itself is a nice square hardback. The instructions are very clear and easy to follow. My five year old son and I easily built the gingerbread house together (pictured above, with a few color swaps). I’d say a typically eight year old could do most of the models with only minor assistance.

My only complaint, and really this goes for most of the LEGO books, is the challenge of getting the right pieces. Most models in the book require at least a couple of specialty pieces, in most cases multiples and in specific colors. The average child simply won’t have enough of these pieces to build a model. Even I (an adult with decades of LEGOS) had to substitute a few pieces for ones I didn’t have.

In short, few people have the exact pieces to build these models without specifically ordering them for the project from a website like BrickLink and BrickOwl, or the rare times that a LEGO Store’s Pick a Brick wall has what you need.

Chris McVeigh has amazing skill and artistic ability in creating Lego creations that are sturdy and beautiful. No Starch Press makes beautiful books for the AFOL who has a Lego Book Library.

This book is amazing and just in time for the holiday Lego prep! Now to bricklink to get the bricks and plates needed to make some of these amazing ornaments.

My only wish is that No Starch Press and Chris McVeigh team up for even more great Lego books!

This is the second book in the Sex and the Season Series and is about Lily's sister Rose. Rose has always been the good daughter. She does everything that is expected of her. Never doing anything that would be inappropriate. Then she meet Cameron Price. Lily's husband asked Cameron to write a waltz for his and Lily's wedding in the first book. He also asked that Cameron work with Rose so that she could play it at the wedding. Rose fell for Cameron while working with him and him for her. However Cameron doesn't believe they could ever be together. He is just a common boy and she is a Lady. He points it out ever time he sees her. Then at Lady Lily's wedding something changes. He decides to let Rose know how he feels. This wakes something up in Rose. She has never felt this way about anyone before. Rose has been seeing Lord Xavier but after her experience with Cameron she realizes that she doesn't want to be with him anymore. He doesn't make her feel the some way Cameron does. While Xavier would be a great match for her and give her everything she ever wanted, she can no longer settle for just that when she knows what it could be like. Will Rose and Cameron find a way to be together or will they be destined to love someone they can never be with.

I loved this book as much as the first. Rose and Cameron go through so much so that they can be together. They fall in love in a time were it isn't appropriate for them to be together. There are twists and turns that make for a great ending. Rose goes through a lot trying to decide if she wants to do what is expected of her or what her heart wants her to do. She envies Lily and her ability to do what she wants regardless of what others think of her. She wants a great love and to not settle. Another great book by Helen. I look forward to reading other books in this series as they come out.

I loved ‘Lily and the Duke’; the first book in Helen’s Historical Romance series, so when I got asked if I’d like to read and review ‘Rose In Bloom’ I practically jumped at the chance….And Helen did not disappoint.

She writes these books with such grace and finesse, that you feel as though you’ve been transported back in time and are walking among the characters. You feel every bit of passion, anguish and love emanating from the pages. It envelops you, and leaves you grasping at the hopes that these two, wonderfully in love couple get to have the HEA they both deserve.

We first meet Rose and Cameron when they are thrust together to produce a commissioned Waltz, for the Duke and Lily’s wedding. Cameron being the composer, while Rose performs it on the Pianoforte.

From that moment and the weeks that followed, feelings between the two have been growing beneath the surface. However, with them being from different ‘stations’ it is not the done thing. ‘Rose In Bloom’ is a story that will pull at your heartstrings. You will fall in love with Rose and Cam, as well as the rest of the characters. From their unabashed behaviour and humorous conversations, they are the perfect blend of love, wit and loyalty.

Although, not the best way to do it, I could understand Cam’s reasoning for pushing Rose away. He was sacrificing himself, and his happiness for the sake of his family; “Worry for Kat. Loneliness for Rose. Hatred for himself. No Happiness. No contentment. No peace.” I felt heartbroken for him, and for Rose.

This is a story of two people coming together; defying the conformities of gentility, in hopes that love can conquer. Just about everything stands in their way of happiness, and both Cam and Rose are tested to the max to see whether or not their love can/will survive.

If you love Historical Romances, then you are sure to love this book….I know I did.

Sigh….Hardt’s books are my undeniable guilty pleasure. So this book starts a few weeks after Lily and Daniel have become engaged. If you read the first book then you know Daniel had just commissioned the sexy commoner Cameron Price, a tenant on his land, to compose a song for Lily as a wedding gift. This commission places Rose, Lily’s sister, in close quarters with the handsome yet somewhat bitter Cam as she is a musician who will be playing the song on the pianoforte.

Over the course of the few weeks that Rose has spent with Cameron there is a magnetic chemistry between them shimmering beneath the surface. However Cam’s disdain for the upper class and his station in life has skewed not only his perception of what could be with Rose but has also pushed her away from him. One fateful night when the two are brought together under the stars the attraction and love for one another finally breaks free. Soon both Cam and Rose find that their love for one another will put them to the ultimate test, will it survive the strain of monetary stations or the rival pursuer of Rose’s affections? Will their love remain faithful or will class divide them so far pushing them into the arms of another? Well you will just have to read and find out, no won’t you ;)

I love Helen Hardt’s regency stories. Her stories are heavily focused on testing the strength of love between two people, throwing just about anything she can to stand in the way and seeing how the two characters can rise above it. It provides just enough angst to get the reader riled up, just enough emotion to make you weep and just enough unfiltered love to make you melt. AND let’s not forget the sexy as hell love scenes in this book, talk about HOT! Some of the scenes are scorching and will totally get your knickers in a bunch. Hardt’s characters are lovable, genuine and perfect. You will fall in love with their brazen behavior especially from the ladies and their never ending wit. If you are a lover of regency romances I suggest you pick up this book now, you will not be disappointed!

A great resource for the novice or experienced NP!

Although the initial chapter on Clinical Reasoning and Differential Diagnosis is limited, it is a concisely written prelude to a wealth of focused symptom assessment strategies. As a practitioner and a faculty member in a nurse practitioner program, I have found this book to be an excellent teaching resource. Each chapter contains several of the most common symptoms related to a specific body system. For example, the cardiovascular and respiratory systems are represented by the symptoms of dyspnea, chest pain and cough. The overlapping of these systems is artfully woven together is a manner that consistently prompts the reader to be focused yet complete. Each chapter outlines key questions to ask and also provides analysis of the possible patient responses. Included is a relational inference between each symptom and age specific concerns. At the end of each chapter, the authors have developed "Differential Diagnosis" tables that are particulary useful to the novice nurse practitioner in a clinical setting. Overall the book's pocket size format lends it to easy reading and convenience.

Comparison to 2nd edition

Nice book. I have the 2nd edition and compared it with the newer edition. The 3rd edition is much the same as the 2nd but with 2 additional chapters--one on "Mood Changes, Behavior Changes..." and then one on clinical CXR (chest x-rays). Helpful book for students.

Excellent book for practical clinical advice...

I am studying to become a Family Nurse Practitioner through Georgetown and absolutely love this book! It has by far the best practical clinical applications of any assessment textbook that I have ever used. I haven't even really used our other textbook in favor of this one. Easy to read- no fluff at all.
